Large sprung/split washer, holds light pressure on bolt so it wont(pray) come loose

Hey all,

I have a 78 civic cvcc and the bottom bolt keeps coming loose and actually fell out of the front right brake caliper. When I took the caliper all the way off I found that the threads on the frame member (the piece that the caliper bolts onto) looks like they are a little mushroomed towards the fron so I tapped the hole, put in a replacement bolt (a $7 bolt that the honda dealership very graciously gave me for free) and loaded it up with thread lock on and put it in but it wiggled its way out, recently on the highway the top on fell out and the caliper popped off, which wasn't fun at all. Any ideas?
